The Housing Navigator will identify and build rapport with housing providers in the Cape Cod CoC so that affordable permanent housing solutions can be located for the homeless individuals at the St. Joseph Shelter or within the Cape Cod CoC who are seeking services. The Housing Navigator will assist clients in breaking the cycle of homelessness by helping identify housing affordable housing solutions so that clients can rapidly obtaining permanent housing. The Navigator will provide individualized client support throughout this entire journey by helping each client address the barriers to obtaining and maintaining permanent housing identified by the client?s individualized service plan developed with the client?s Case Manager. This may include linking with appropriate medical needs housing, substance use recovery housing, Department of Mental Health housing, local Housing Authorities and other affordable housing providers within the Cape Cod CoC. As part of the plan, the Navigator in conjunction with the client will identify each area the client may need assistance with to accomplish the outlined goals and objectives. The Housing Navigator will work closely as part of the PATH outreach team to track homeless trends and work with the community to identify solutions

Facilitate in-person discussions with individual experiencing homelessness.
Ensure access barriers are effectively addressed (i.e. translation services, transportation issues)
Facilitate creative brainstorm around housing options each family may have.
Utilize strengths-based philosophy and motivational interviewing skills when working with families in crisis.
Utilize mediation and conflict resolution skills to assist families with relational challenges.
Complete comprehensive diversion training, including mediation and conflict resolution.
Clearly articulate housing limitations and encourage self-determination and resourcefulness.
Make referrals for financial assistance for clients when necessary and appropriate.
Provide reminder calls and emails to people with scheduled appointments as appropriate.
Complete release of information, client rights, and HMIS paperwork with each client.
Be familiar with shelter and housing resources within the CoC system.
Clearly articulate where clients are in the process and answer any questions client have.
Clearly and compassionately inform clients if they do not or no longer qualify for assistance.
Provide additional resource information to clients as appropriate.

General Responsibilities:
Adhere to confidentiality guidelines and respect privacy of clients accessing services.
Ensure the individual voice and choice are represented within assessment data and service plans.
Ensure records are kept in accordance with agency standards.
Complete initial training with supervisor.
Participate in staff and supervisory meetings as required.
Contribute to and support a positive, team-oriented, culturally diverse work environment.
Perform other job-related duties as assigned.

This position requires the employee to work in an environment where there is a high level of pressure to divert from or place families in shelter quickly; there may be times when working conditions include interruptions and interactions with family members who are angry/upset and/or dealing with mental health and substance abuse issues. Extensive use of telephones, computers and related office equipment will be required. Travel between assessment offices may be required.

Degree in social services or a related field, equitable experience in social services.
Ability to answer phones and provide consistent and timely information to a diverse population.
Understanding of issues homeless clients may experience, including alcohol and drug abuse, mental illness, domestic violence, etc. Understanding of barriers homeless immigrant and refugee families may experience.
